Dapoxetine or Skill Training: What the Difference Is
An honest comparison of dapoxetine and ejaculation-management training: what the pill gives you, what side effects it has, and why skill works over the long run where chemistry does not.
Updated:
When a man first goes to a doctor with PE, he often gets a prescription for dapoxetine. It is a short-acting SSRI taken 1-3 hours before sex. In clinical studies it really does increase time to ejaculation - on average 2 to 3 times compared with placebo. That effect is real. But there is a catch: dapoxetine does not teach the body anything. Remove the pill and the pattern returns. Then there are the side effects: nausea, dizziness, diarrhea, apathy, lower libido, and for some men a loss of sensation during sex. Let's look at when the pill makes sense and when skill works where chemistry is powerless.
In short
- ✓Dapoxetine affects the symptom through brain chemistry, but it does not retrain the nervous system. After discontinuation, the pattern returns.
- ✓Documented side effects of dapoxetine include nausea (in 11-20% of users), dizziness, diarrhea, apathy, and lower libido.
- ✓Skill training through the Bottle Model and conscious practice creates gradual but stable change in the pattern without chemistry and without side effects.
- ✓A combination is possible: the pill can act as a temporary safety net while the skill is still forming, but that should be discussed with a doctor.
- ✓The main question is whether you want a one-time effect today or a skill that stays with you forever, with any partner and in any situation.
What dapoxetine does and why it is a temporary crutch
Dapoxetine is the only SSRI approved specifically for treating PE. It works quickly, within 1-3 hours, and leaves the body within 24 hours. Its mechanism is simple: it increases serotonin in the synaptic cleft, which inhibits the ejaculatory reflex at the spinal level.
The problem is not that the mechanism does not work - it does. The problem is that the brain and body do not receive any new experience. The nervous system does not learn to notice the early signs of PONR. Breathing stays the same. Pelvic tension stays the same. Anxiety stays the same. As soon as the substance wears off, everything returns to the starting position.
It is like painkillers with a broken leg: they reduce pain but they do not fuse the bone. In PE, that "bone" is the neural arousal pattern the body has learned over years. A pill does not change that.
The side effects nobody talks about in the ads
The official prescribing information for dapoxetine lists nausea (in 11-20% of patients), dizziness, diarrhea, insomnia, and increased sweating. There is also the sexual side of the story: lower libido, reduced orgasm intensity, and for some men a kind of numbness in sex. So sex lasts longer - but feels less alive.
There is also serotonin syndrome when it is combined incorrectly with other drugs - rare, but real. And there can be syncopal episodes, especially when mixed with alcohol, which the manufacturer does not recommend even though people do drink in real life.
For a doctor who sees a patient for 15 minutes, a dapoxetine prescription is a fast and justified decision. For a man who wants normal sex without pills, it is a starting point, not the finish line. Skill is the finish line.
What training gives you where the pill cannot reach
Training through the Bottle Model, conscious masturbation, and breathing work changes the pattern from the inside. A man learns to notice rising arousal earlier, manage the temperature of sensation, and open the leak through relaxation. This does not happen in one day, but each repetition lays a new track into the nervous system.
The long-term difference looks like this: Pill: works today -> does not work without the pill Skill: builds slowly -> works always, with any partner After 30 days of regular practice, most men notice their first stable changes. Not records, but a different quality of presence in sex.
Another point people often miss is anxiety. Dapoxetine does not reduce fear of failure, remove shame, or change a man's relationship to his partner. He takes the pill and still tightens up from fear that "what if it doesn't work today." Working with anxiety means working with the body and attention, not just chemistry.
When the pill is still justified
There are situations where dapoxetine is reasonable: very high anxiety that interferes with practice, a situation where short-term relief is needed, such as a new relationship or an important encounter, or as a temporary safety net while the skill is not yet stable. In such cases, it should go through a doctor.
An important point: the Zhurov Method is an educational skill-training system, not treatment. We do not replace doctors. Whether to take dapoxetine is a medical question. We are talking about training the pattern, not prescribing medication. If you are unsure or have medical symptoms, start with a urologist.
The short conclusion is this: the pill and the skill do not exclude each other. But if you want a result that stays with you without chemistry, training is required. And that is possible.
First step for 7 days
- •Write down honestly what you want: a one-time effect today or a skill that stays with you. Those are different strategies.
- •If you are currently taking dapoxetine, continue following your doctor's advice. In parallel, begin one short CM session per week with attention on the Bottle. The skill can be built alongside medication support.
- •After one sexual encounter with the pill and one without it, compare not only "how many minutes" but the quality as well: was there contact with the body, was there presence, did the sensations feel alive?
- •Discuss with your doctor the possibility of gradually lowering the dose as the skill forms. That is a normal conversation to have with a medical specialist.
Common questions
Is dapoxetine safe?
For most men without contraindications, it is relatively safe when used correctly. But it has real side effects and contraindications. It is a prescription drug, so the decision should be made together with a doctor, not by buying it from a pharmacy without proper guidance.
Can I train and take pills at the same time?
Yes. Some men use dapoxetine as a temporary safety net during the first weeks of training while the skill is not yet stable. That is a reasonable strategy when coordinated with a doctor.
Why is skill better than a pill over the long run?
Because skill transfers everywhere: a different partner, a different situation, a different mood. A pill only works while you are taking it. The skill stays with you.
Is this medical treatment?
An important point: the Zhurov Method is an educational skill-training system, not treatment. We do not replace doctors. If you have acute physical pain, inflammation, or other medical symptoms, start with a urologist. That is a normal adult step for a man, and we can train attention management and relaxation in parallel.
